Payment/Refund Issues

Refund policy:  All refunds must be approved by the Department of Public Health.

Result/Document Issues

Licenses will be issued by the Department of Public Health after completion of all required processing.

Login Issues

If the user does not know the MCID or Service ID number, create a Further Action Required ticket providing the Provider's name, what they believe their MCID and Service ID are, as well as contact information for a return call.
