Payment/Refund Issues

Refund policy: 

All refunds must be approved by the Department of Public Health.

Result/Document Issues

Licenses will be mailed to the licensee by the Department, after the completion of all required processing.

Login Issues

If the user does not know the license number or last 4 digits of the social security number, create a Further Action Required ticket providing the licensee's name, what they believe their license number and last 4 of social are, as well as contact information for a return call.
